Schizophrenia is a chronic mental disorder requiring strict management and continuous monitoring of treatment. Adherence to follow-up appointments is a key determinant in preventing relapse and minimizing hospital readmission rates. This study aimed to analyze the relationship between family support and adherence to follow-up appointments among schizophrenia patients at the General Outpatient Clinic of the Dr. Arif Zainuddin Regional Psychiatric Hospital (RSJD) in Surakarta. This was a quantitative, correlational study using a cross-sectional approach. The study population comprised all outpatients with schizophrenia; a sample of 125 respondents was selected using purposive sampling based on inclusion and exclusion criteria. The instruments used included questionnaires on family support and adherence to follow-up appointments, both of which had undergone validity and reliability testing. Data were analyzed using univariate and bivariate methods, specifically the Spearman Rank correlation test. Univariate analysis results showed that the majority of respondents were in the 40–59 age group (36.8%), male (51.2%), and unemployed (37.6%). Most patients received a moderate level of family support (44.8%) and demonstrated moderate adherence to follow-up appointments (39.2%). Bivariate analysis revealed a significant positive relationship between family support and adherence to follow-up appointments (p < 0.001; r = 0.490). Optimal family support plays a crucial role in improving treatment adherence among patients with schizophrenia.
