Tatalaksana Kuretase Gingiva pada Pasien Hipertensi dengan Kebiasaan Merokok dan Konsumsi Alkohol

Abstract

Background: Periodontitis is an inflammatory disease of the periodontal tissues that can lead to destruction of the supporting tissues of the teeth if not properly treated. Periodontal therapy is performed in stages, beginning with initial therapy consisting of scaling and root planing. Gingival curettage may be performed as an adjunctive therapy when the initial treatment does not result in optimal clinical improvement. Objective: To discuss the management of gingival curettage in a patient with hypertension and a history of alcohol consumption and smoking. Case Report: A 29-year-old male patient presented to Soelastri Dental Hospital with a chief complaint of frequent gingival bleeding during toothbrushing. Clinical examination revealed a blood pressure of 138/90 mmHg, an OHI score of 2.6, a plaque index of 56%, and a gingival index of 1.5. Probing depth on tooth 31 was 4 mm at the mesiolabial site, 3 mm at the midfacial site, and 4 mm at the distolabial site. The patient had a history of hypertension, smoking habits, alcohol consumption during periods of stress. Results: Post-treatment evaluation showed reduced gingival swelling, and the patient no longer reported gingival bleeding during toothbrushing. However, a 4-mm periodontal pocket with positive (BOP) was still present. Conclusion: Gingival curettage as an adjunctive treatment following initial periodontal therapy resulted in clinical improvement of the periodontal condition. The success of periodontal therapy may be influenced by systemic conditions and behavioral risk factors, including hypertension, alcohol consumption, and smoking.
