Abstract

Background: Failure to achieve exclusive breastfeeding is frequently associated with insufficient breast milk production during the early postpartum period. As a non-pharmacological intervention, the SPEOS method, which combines endorphin massage, oxytocin massage, and positive suggestion, has the potential to enhance both physiological stimulation and psychological well-being, thereby promoting breast milk production. This study aimed to evaluate the effect of the SPEOS method on improving breast milk production among postpartum mothers. Methods: This quasi-experimental study employed a nonequivalent control group design involving 20 postpartum mothers. Participants were recruited using purposive sampling and allocated into an intervention group (n = 10) and a control group (n = 10) with 1:1 individual matching based on parity. The intervention group received standard postpartum care combined with SPEOS therapy for 15–20 minutes once daily for three consecutive days, while the control group received standard postpartum care only. Breast milk production was assessed on day 1 (pre-test) and day 4 (post-test) using the Composite Breast Milk Production Observation Instrument. Data were analyzed using the Mann–Whitney U test after the Shapiro–Wilk test indicated a non-normal distribution (p < 0.05). Results: Descriptive analysis demonstrated that the mean breast milk production score in the intervention group increased from 6.20 at baseline to 11.70 at the final assessment, with a median gain score of 6.00. In contrast, the control group showed only a slight increase from 6.20 to 6.60, with a median gain score of 1.00. The Mann–Whitney U test confirmed a highly significant difference in improvement between the two groups (U = 0, Z = −3.78, p < 0.001). The intervention also produced a large effect size (r = 0.85). Conclusion: The SPEOS intervention significantly improved indicators of breast milk production among postpartum mothers. This complementary approach has the potential to be an effective intervention for supporting breastfeeding success in primary midwifery care.
